To begin the process:

✓Speak with your care coordinator to arrange a financial assessment.

✓If eligible, you’ll receive a personal budget, which may be provided as a Direct Payment.

✓A care broker will then help you find the most suitable care package.

Needs Assessment & Next Steps

Once funding is confirmed:

  • We’ll arrange for a manager to visit you, along with your care coordinator or a family member, to complete an initial needs assessment.

After the assessment, you’ll receive our Service User Guide Handbook, which outlines how your care will be delivered and how we ensure quality and consistency.
