When you realised that you needed care, you may have approached us directly; alternatively, you may have been referred to us by the local authorities/health service and which might be funding some or all of your care.
Information about you which is passed to us will be dealt with sensitively and in confidence.
Step By Step Process
Schedule a formal assessment.
We learn more about your likes and dislikes
Talk with the family and understand their expectations for the family member.
Discuss and develop a personal care plan for their loved one.
Conduct a Needs Assessment – Assess the loved one and their environment to develop a care plan suitable for their environment.
Develop a weekly schedule based on the care needed and critical times.
Discuss Unicorn Healthcare Services policy and procedures, service user rights and responsibilities, emergency plan, and complaint procedures.

Safeguarding Report Form
When reporting safeguarding issues we always do so with the best intent. Once assessed any referal may be passed to a supporting authority to best safeguard a Service User or an indivudual. We accept that as part of this process we may be required to provide statements and / or evidence to outside agencies and we support this readily.
Safety is paramount to all we do.
Personnel Spot Check
This spot check will be conducted at the service user’s residence, providing an opportunity for them to provide feedback on the service received.
Service User Review Form
This Service User reviews help improve service quality, enhance the user experience, and ensure regulatory compliance. By gathering feedback, we can identify strengths, weaknesses, and areas for improvement. This allows us to tailor services to individual needs, and address concerns.
7-Day Client Review
A 7-day client home care review is a comprehensive assessment of a client’s care needs and the effectiveness of the care provided over a 7-day period
Consent Form: Care and Treatment
This form can be used to record agreements with the person using the service about their planned care and treatment in general and/or specific aspects not covered by their general agreement that requires separate consent. It assumes that people can give their consent unless it is proved from assessment that they lack the capacity, in which cases best interests decisions must be taken.
Weekly Medication Audit Report
A weekly medication audit report is a crucial tool for ensuring the safety and efficacy of medication administration to our Service Users in homes.
Care and Support Plans: For Co-ordinating & Auditing
Compliance assessments will nearly always include scrutiny of the care plans produced by the service in terms of whether they are person-centred, comprehensive, up to date, accurate and sensitive in terms of recording, etc. This form should help our managers with their monitoring and auditing to check that their care plans comply with the quality standards. The aim should be to audit each plan regularly, such as every three or six months.
