When reporting safeguarding issues we always do so with the best intent. Once assessed any referal may be passed to a supporting authority to best safeguard a Service User or an indivudual.  We accept that as part of this process we may be required to provide statements and / or evidence to outside agencies and we support this readily.

This spot check will be conducted at the service user’s residence, providing an opportunity for them to provide feedback on the service received.

Consent Form: Care and Treatment

This form can be used to record agreements with the person using the service about their planned care and treatment in general and/or specific aspects not covered by their general agreement that requires separate consent. It assumes that people can give their consent unless it is proved from assessment that they lack the capacity, in which cases best interests decisions must be taken.

Care and Support Plans: For Co-ordinating & Auditing

Compliance assessments will nearly always include scrutiny of the care plans produced by the service in terms of whether they are person-centred, comprehensive, up to date, accurate and sensitive in terms of recording, etc. This form should help our managers with their monitoring and auditing to check that their care plans comply with the quality standards. The aim should be to audit each plan regularly, such as every three or six months.
